CHP’s focus is on transforming community health in the Pikes Peak region. We work to disrupt the ways we work together to solve complex community issues. We don’t attempt to solve health equity issues through direct services. Instead, we bring together diverse groups of people who represent organizations and communities to work on changing the conditions that hold a problem in place. We are containers of change, we build movements, and we build collaborative governance structures to build collective community power to influence change. This annual report offers a few of the highlights of how CHP helped the community access a wide range of resources and ideas leading to better health outcomes for our region.
